About Fibroid

Fibroids are non-cancerous growths in the uterus that can cause pain, heavy bleeding, and other symptoms.

  • Fibroids are benign tumours arising from the muscle of the uterus
  • They are also called myomas or leiomyomas
  • Fibroids occur in 30-40% of all women
  • They may be located on the surface of the uterus (serosa), deep in the muscle (myometrium) or within the cavity of the uterus (endometrium)
  • They may be single or multiple

Symptoms

Symptoms of Fibroids

  • Majority of the fibroids are small usually less than 5cm and asymptomatic
  • However as they grow or come into the uterine cavity, they may cause symptoms
  • They may be located on the surface of the uterus (serosa), deep in the muscle (myometrium) or within the cavity of the uterus (endometrium)
  • They may be single or multiple
  • Heavy uterine bleeding
  • Severe pain during menses
  • Pressure on urinary bladder causing increased frequency & urgency of passing urine or sometimes obstruction to passage of urine
  • Pressure on bowel causing increased frequency & urgency of motions or sometimes constipation & bowel obstruction
  • Feeling of heaviness & lump in abdomen
  • In pregnancy, it can lead to premature labour & delivery, abnormal position of the baby and obstruct delivery with increased chances of Cesarean delivery
  • It can undergo degeneration (red degeneration) causing severe pain and fever
  • However, if a fibroid is diagnosed during pregnancy, pregnancy should be continued under supervision and does not require termination

Treatment

Treatment options for Fibroids

Overview

Small fibroids less than 5cm and not causing symptoms do not need treatment. They should be observed at periodic intervals to assess change in size or number

Non-Surgical Methods

  • Medical treatment including injectables (GnRH analogues) and oral medication (Ulipristal, mifepristone, progestins & oral contraceptive pills)
  • High Frequency Ultrasound
  • Embolization of the blood supply of the fibroid
  • Injectable medical treatment can be administered for a short period (3-6 months) prior to surgery in anemic women, in the perimenopause and in case of associated diseases such as endometriosis & adenomyosis
  • Due to severe side effects long term use is not advised
  • Oral medication shrinks fibroids by about 50% and may be used over longer terms of a few years safely especially in perimenopausal women and young women to delay or avoid surgery
  • Ulipristal has been banned in some countries due to the possibility of liver damage
  • High frequency ultrasound (HIFU or MRGFuS) has been used to burn the fibroids under MRI guidance
  • It reduces the fibroid volume and symptoms by 50-70%
  • However regrowth of surviving fibroid tissue can occur, there is no tissue for histopathology and complications such as bowel & bladder burns and nerve injuries have been reported
  • Uterine artery embolisation blocks the blood supply to the fibroids and achieves similar reduction in fibroid volume & symptoms as HIFU
  • However endometrial damage & ovarian failure can occur
  • HIFU & embolization are not recommended in women desiring pregnancy and may not work in multiple fibroids

Surgical Treatment of Fibroids

  • Fibroids need to be removed surgically if they are causing symptoms or in case of failure of medical treatment
  • Laparoscopy is the gold standard for removing most fibroids
  • The whole operation is performed in 3-4 small incisions less than 1cm
  • The fibroids are removed from the uterus and the incisions on the uterus sutured
  • Since there is possibility of spillage of the fibroid tissue in the abdomen, the fibroids are placed in a large and removed in strips (morcellation) by an instrument (Morcellator) introduced in the bag
  • Hysteroscopic surgery (surgery through the uterine cavity) is utilised when the fibroid is not too large and projecting through the uterine cavity
  • The fibroid is cut with an electric current or a saving device into small chips which are removed with the hysteroscopic fluid
  • This procedure allows select fibroids to be removed without any cut on the body and painlessly
